How to repair leaking flat concrete roof ?

This leak is on a carport flat concrete roof measuring 20 feet by 20 feet. Widespread hair line cracks are visible probably due to improper curing (curing too fast without proper 'wet soaking'). Signs of previous repair is visible and the leaks are at these spots as well apart at several other spots. Need advise. Tq.
